About Telepatia

Telepatia builds AI for healthcare in Latin America. Our AI medical scribe is live in Brazil, Colombia is connected, and Chile and Argentina are next. We're AI-first, we move fast, and we care a lot about the work. The integration team has grown quickly: in the last few months we went from 2 to 10 developers, launched 5 new products, and we have already surpassed the million mark for consultations recorded via the platform.

The role

You'll work on our data platform: a lakehouse that ingests every event across Telepatia at high volume, processes it through streaming and batch pipelines into FHIR, the international standard for health data, and serves it to hospitals, clinics and partners through one clean interface. On the same foundation, we have an internal analytics platform so business teams can work with data on their own.

This is a product: it has to scale with growing data volumes, serve clients, stand on its own commercially, and run across several countries from day one. As a founding engineer, you'll design and build it end to end, and be the engineer client IT teams work with to connect.

What you'll do

Design and build the backend services and event-driven architecture behind the data platform, handling millions of records
Work with the PM to shape the platform into a product that scales and that clients can adopt easily
Meet IT teams at client organisations to understand their systems: databases, cloud setup, VPNs, how their teams are organised, and what they need to integrate
Explain the value of the platform to technical stakeholders, not just clinical users
Help build the internal analytics platform that business teams will use every day
Look for improvements and push for them. We want someone who asks "what about this?"
Build with Brazil, Colombia, Chile, Argentina and future markets in mind
Travel to meet clients in person from time to time

What we're looking for

7+ years of hands-on software engineering experience
You still write code. This is a senior role, and you'll be shipping production code yourself
A deep understanding of event-driven architecture and event bus patterns
Strong backend skills in TypeScript/JavaScript and/or Python. Rust is a big plus
Experience with PostgreSQL and MongoDB in production at scale
Working knowledge of Docker and Kubernetes. Our infra team runs the platform, but you should know your way around it
Enough frontend knowledge to handle small frontend tasks yourself
You enjoy talking to clients. You're comfortable running meetings with technical stakeholders, gathering requirements, presenting, and working across teams
A track record of building products from zero to one at startups or strong engineering teams, working inside a team and owning real outcomes
You use AI tools in your daily work
Portuguese and English at a professional level. Spanish is a strong plus

Nice to have

Experience with FHIR, HL7, or other healthcare interoperability standards
Experience building integration platforms, public APIs, or developer-facing products
Experience in solutions engineering or technical consulting with enterprise or regulated clients
Experience mentoring or leading engineers
